Chronic Conditions

Chronic conditions are long-lasting health issues—such as heart disease, diabetes, arthritis, and chronic lung disease—that persist for a year or more, require ongoing medical care, and can limit daily activities.

The Impact in Nebraska

Chronic conditions are among the leading causes of illness, disability, and death across Nebraska. Conditions like high blood pressure, diabetes, and arthritis affect hundreds of thousands of adults statewide, impacting day-to-day quality of life for individuals and their families.

Why Management Matters

While chronic conditions are rarely "cured," they are largely manageable. Taking an active role in managing symptoms, understanding medications, and building healthy daily habits can drastically reduce complications, lower the risk of hospitalization, and help residents maintain their independence and well-being.

How We Support Our Community

At the Sarpy/Cass Health Department, we are committed to helping residents live healthier, more independent lives. We work alongside community partners to bridge the gap between diagnosis and daily management by offering evidenced-based programming, resource navigation and connections, and community education and prevention.
